New Dental Patient Information
Your First Visit
If you’re brand new to the Marlton, NJ dental office of Dr. Kristin Petulla and Dr. Jennifer Kazemi, we’d like to start by welcoming you, and thanking you for trusting our team with your smile. We’d also like to take the time to thank anyone who referred you to our practice, so please let us know about any referrals. At your appointment, we’ll start by reviewing your new patient information forms to ensure we have a complete understanding of your current and past dental health and medical history, as well as any goals you have for your continuing care. The hygienist will update x-rays as needed, carefully examine any old dentistry, and check your gums by completing periodontal charting. She will also do an oral cancer screening, and clean your teeth. Before you leave, one of our doctors will do a complete oral exam and then discuss any areas of concern. If multiple appointments are needed, we will put together a treatment plan for you in order to establish a plan for ongoing care.
New Patient Form
Before your appointment, we encourage you to complete your new patient form from the comfort of home. This documentation is an essential part of your first appointment. We’ll review them with you in order to provide personalized treatments from day one. If you have any questions while completing your form, please don’t hesitate to contact our team.
Insurance & Financing
If you have dental insurance, please let us know. You’ll need to provide this information as part of your new patient form. Our patient coordinators will be happy to review your coverage and help you maximize the benefits of your individual plan. We will also file your claim forms for you on the day of your treatment. For patients needing extensive dental treatment, we offer several payment options, including low to no-interest financing plans through Lending Club and Care Credit.
Patient Referral Program
New patients are always accepted in our office. The greatest compliment our patients can give us is the referral of their friends and loved ones. If you refer someone to our practice, when they come in for treatment, you will receive a $25 credit on your account as a thank you and token of our appreciation.
